Why this matters
The trend charts only exist because these fields are filled in appointment after appointment. Weight gain, the blood-pressure curve and fundal height in series catch a deviation before the symptom appears — something free text never delivers.

- Write the plan in Observações in short sentences — that is what you will re-read next time.
- Look at the chart before you close: the trend says more than today's isolated value.
